Administrator of the Month
Nomination Form
This is a program of the Student Government Assembly

Purpose:
For the student body to recognize administrative employees for excellence in providing services to students and the college community. This program is designed to increase the morale of the administrative body, recognize their service to the college and develop a stronger bond between students and administrative employees.

Criteria for nominations:
2 general areas.
Service to students:
*availability and accessibility to students
*Demonstrated caring about students and campus community
*High level of responsiveness to student requests
*Working hard to make campus community better as a whole (doesn't have to be a direct service to students).
*reliability in providing accurate information
*Demonstrates Commitment to the job and providing services to students

Customer service skills:
*Demonstrates willingness to Follow up on requests
*Is able to provide a high level of Knowledge and resources on college policy and *Demonstrates Quick turn-around time in processing requests

Nomination process:
In 500 words or less state how the administrative employee has exemplified the above criteria in the quality of customer service received and in service to the student body you feel the Administrator exemplifies.

All nominations are to be received by the first day of each month. Though nominations will be received on an ongoing basis, nominations received by the first of the month will be considered for that month only.
You may use the back of this form to submit your nomination. All nominations are to be filed with the Student Government Office in Tivoli 307 in print or can be e-mailed to administrative-nominations@lists.mscd.edu
