For full time and part time faculty
Nomination Form
This is a program of the Student Government Assembly

Purpose:
For the student body to recognize full time and part time faculty for excellence in advising and teaching. This program is designed to increase the morale of the Faculty body and create greater bonds between the faculty and student bodies.

Criteria for nominations:
Two general areas.
Advising:
*Accessibility
*Approachability
*Completeness and accuracy of information
*Caring/compassion

Teaching:
*Organization of course material
*Ability to communicate
*Effective use of class time
*Showing enthusiasm for subject matter and parting this enthusiasm to students
*Tutoring
*Relevance of text vs.course material

Nomination process:
In 500 words or less state how the faculty member has exemplified the above criteria in the quality of education you receive and interaction with the faculty member in and outside of the classroom you feel the faculty member exemplifies.

All nominations are to be received by the first day of each month. Though nominations will be received on an ongoing basis, nominations received by the first of the month will be considered for that month only.
You may use the back of this form to submit your nomination. All nominations are to be filed with the Student Government Office in Tivoli 307 in print or can be e-mailed to faculty-nominations@lists.mscd.edu
