Student Employee of the Month
Nomination Form
This is a program of the Student Government Assembly

Purpose:
For the college at large to recognize student employees for excellence in customer service skills and job performance. This program is designed to increase the morale of the student body and to recognize fellow students for their service to the college.

Criteria for nominations:
Two general areas.
Job performance:
*Is Dependable and reliable
*Has Initiative, proactiveness, and is progressive with job duties
*Demonstrates Commitment to the job

Customer service skills:
*Provides a high level of Accurate information
*Demonstrates willingness to Follow up on requests
*Is able to provide a high level of Knowledge and resources on college policy and

Nomination process:
In 500 words or less state how the Student Employee has exemplified the above criteria in the quality of job performance and customer service you feel the Student Employee exemplifies.

All nominations are to be received by the first day of each month. Though nominations will be received on an ongoing basis, nominations received by the first of the month will be considered for that month only.
You may use the back of this form to submit your nomination. All nominations are to be filed with the Student Government Office in Tivoli 307 in print or can be e-mailed to student-nominations@lists.mscd.edu
