If you would like to change from non-degree to degree seeking or vice versa, complete and submit this form to the Office of Admissions (you can also fax it to 303.556.6345).

Student Name Change Form ( 7 KB)

If you have changed your name since the last time you attended MSU Denver, you will need to submit this form with the appropriate legal documentation in order for us to finish processing your admissions application.

Transcript Request Form ( 27 KB)

You can send this form to other schools to request your official transcripts to be sent to MSU Denver.

In-State Correction Form ( 245 KB)

If you forgot to include some residency information on your admissions application, use this form to add or correct it.

High School/GED Information Form ( 39 KB)

If you forgot to include your high school/GED information on your admissions application, use this form to add it.

Semester Change Request Form ( 45 KB)

 If you need to change your semester entry term  for enrollment, use this form.

Parent Signature and Application Information Correction Form ( 166 KB)

 If you are under 18 and you and/or your parent did not sign your application for admission, fill out this form, sign, and submit it to the Office of Admissions.

Colorado High School/GED Non-Resident Tuition Rate Affidavit ( 256 KB)

 If you qualify for the Colorado High School/ GED Non-Resdident tuition rate, this affidavit must be submitted by the application deadline for the semester you are applying for.
