to the Office of the Registrar, CN 203, along with payment. |

In order to have a faculty members consecutive classes
scheduled back-to-back in the same classroom, chairs should
complete this form and have the request approved by the dean
of the appropriate school and by the Office of Academic Affairs.

CAPP
Adjustment Form |
An approved CAPP Adjustment Form is required whenever a degree
requirement indicated on a student’s CAPP Report is to
be modified. The form is usually completed by the student’s
advisor or appropriate academic department chair, and must be
signed and submitted to the Office of the Registrar, CN105 by
the department chair or designee. It is highly recommended that
approved adjustments be made early in the student’s academic
career. |
| Certificate
Program Plan Form |
Certificate programs provide opportunities to successfully
complete a series of five to eight academic credit courses that
focus on a particular area of career ineterest. Each certificate
program is designed to stand alone or merge with your degree
program major or minor. The certificate title and date of award
will appear on your official transcript. The certificate program
is coordinated by the Office of Academic Affairs, 303-556-3040.
Students must complete each course in the certificate program
with a grade of "C" or better. These courses may not
be taken pass/fail. |

To adjust teacher licensure requirement only in CAPP. Must
be
approved by the chair of the Teacher Education Department.
