Diploma Replacement Order Form Graduates of Metropolitan State College of Denver may order a replacement diploma by submitting the order form to the Office of the Registrar, CN 203, along with payment.
Confidentiality Request: Federal Educational Rights and Privacy Act (FERPA) To insure no Directory Information will be disclosed to anyone without your express written request.
For Faculty/Staff
Back to Back Form

In order to have a faculty members consecutive classes scheduled back-to-back in the same classroom, chairs should complete this form and have the request approved by the dean of the appropriate school and by the Office of Academic Affairs.

CAPP Adjustment Form
An approved CAPP Adjustment Form is required whenever a degree requirement indicated on a student’s CAPP Report is to be modified. The form is usually completed by the student’s advisor or appropriate academic department chair, and must be signed and submitted to the Office of the Registrar, CN105 by the department chair or designee. It is highly recommended that approved adjustments be made early in the student’s academic career.
Certificate Program Plan Form Certificate programs provide opportunities to successfully complete a series of five to eight academic credit courses that focus on a particular area of career ineterest. Each certificate program is designed to stand alone or merge with your degree program major or minor. The certificate title and date of award will appear on your official transcript. The certificate program is coordinated by the Office of Academic Affairs, 303-556-3040. Students must complete each course in the certificate program with a grade of "C" or better. These courses may not be taken pass/fail.
CAPP Adjustment Form - Teacher Licensure To adjust teacher licensure requirement only in CAPP. Must be
approved by the chair of the Teacher Education Department.
Class Adjustment Form To add, cancel, change days/times, or otherwise adjust a class offering.
Correspondence Grade Assignment For use with students who are being assigned a letter grade who were previously assigned a continuing correspondence (CC) grade notation.
Last Grade Stands Form To change all previous attempts of a course to a grade notation of N/C (no credit).
No-Credit(NC)/Withdrawal Form
To receive permission to withdraw from a class up to 2/3 of the way through the semester.
Class/Section Change Form To change a student’s registration from one section of a class to another. Generated by the department
Room Change Request Form To request a room change for a specific class.

To obtain student lists/address labels for mailing for the current semester

Audit Approval Form To attend a class, unofficially, paying only tuition—no fees—with no record maintained at the college.

COF Waiver Request To request a waiver of the 145 COF credit hour limit.

Grade Appeal Petition To appeal a grade.

Name Change Form To change your name on college records by providing a legal name change document: marriage certificate, divorce decree, etc.
Pass/Fail Form To request a Pass/Fail option for a class, as opposed to normal grading options of A, B, C, D, F. This is restricted to general elective classes only. Contact the Office of the Registrar at regquery@mscd.edu to request a copy or stop by the Registrar’s Office in CN105 to pick one up.
Petition for In-State Tuition To petition to change residency status for tuition purposes from out-of-state to in-state.
Tuition and Fees Appeal This form is administered by the Office of Student Accounts, CN 110, 303-556-6188.
