ADMIT

According to our records you have not been admitted to MSU Denver or you have been admitted for a future semester and we cannot complete your file at this time. If you have not completed an admissions application, you should contact the Admission's office in the Student Success Building, Suite 180. If you have completed an admissions application, you may not have been officially accepted for the current semester. Our office will be notified of your acceptance by the Admissions Office and will automatically update this request at that time. To check on the processing of your admissions application, contact the Admission's office in the Student Success Bulding, Ste 180. If you have received your admissions decision, it may just be a timing issue due to your admittance for a future semester and you can disregard this request.

 

 

APP

We have not received record of your Free Application for Federal Student Aid (FAFSA) for the current academic year. If you have already completed the application and listed Metro as a college to receive the information, we will receive the information electronically. To verify when the information will be sent, you may call The Federal Student Assistance Hotline at 1-800-433-3243. If you have not yet filed an application for financial aid, you may complete your FAFSA online at www.fafsa.ed.gov. Paper applications are available in our office or at local colleges and high schools. Reminder that you will not be able to receive any types of aid such as grants or loans if you have not filed and our office has not received your FAFSA.

 

BDATE

There is a discrepancy in your reported date of birth. Please provide us with proof of your birth date, (i.e. birth certificate or driver's license). The deadline to submit this form is 2 weeks prior to the end of the semester if you wish to receive financial aid - failure to submit the documentation or submit it incompletely can result in you not being eligible for aid.

 

 

BNKRPT

Our office has been notified that you are in an active bankruptcy status. If you have defaulted on a student loan and/or have a grant overpayment that is non-dischargeable and was included in your bankruptcy you need to resolve these issues before you are eligible for any additional Title IV aid. In order for your lender to fund any future disbursements, you will need to complete and submit another Federal Stafford Loan Acceptance form. When you file for bankruptcy all future aid disbursements are cancelled. Therefore, any expected disbursement through Metro has been terminated.

 

 

CITIZN

When applying for financial aid, your citizenship status was not confirmed by the Social Security Administration. To be awarded financial aid we must have documentation that you are a citizen or eligible non-citizen. If you are a US citizen, a copy of your birth certificate or passport is required. If you are an eligible non-citizen, you must correct your citizen status on your Student Aid Report and provide your resident alien registration number. The deadline to submit this form is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

 

DEFALT

We have received notification from the Department of Education that you are in default on a student loan. You will not be eligible to receive financial aid until you provide our office with documentation that shows you are no longer in default. You may want to contact the agency which was reported on your Student Aid Report (SAR) to clear your default status. You should also want to contact the U.S. Dept. of Education to clear your status with them. The deadline to submit this form is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

 

DRUG

Please complete the DRUG worksheet that you received from the federal government and determine if your conviction affects your eligibility to receive aid. If you are eligible to receive aid, then you will need to update your FAFSA online. You can also call 1-800-4-FED-AID to get additional help with this question. You are ineligible for financial aid if you do have a qualifying offense or until this issue is resolved. The deadline to submit this documentation is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

 

DSBLTY

The federal government has informed us that you have one or more student loans discharged because of a total or permanent disability. Before you can receive additional Federal student loans for this year, you and your physician must fill out the DSBLTY form (located at) http://www.mscd.edu/financialaid/forms and return it to our office. You will also need to contact your guarantor or loan holder to determine whether or not you need to provide them with a physician statement as well. The deadline to submit this form is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

DSWORK

You indicated on your FAFSA that you, your spouse, or your parents are considered to be either a Dislocated Worker or Displaced Homemaker. Please complete the DSWORK form at http://www.mscd.edu/financialaid/forms and return it to our office.

 

 

 

 

DVER

You were selected for verification. Please complete the DVER form at http://www.mscd.edu/financialaid/forms and return it to our office. Follow the instructions on the form carefully. Many of these forms end up being returned due to no signatures, missing W2's or the packet is not fully filled out. Should you have difficulty filling out the paperwork and need assistance, please contact our office. Please note that if you live away from your family and you don't turn in your lease, mortgage statement, etc - you will receive an at home budget and your file will NOT be considered incomplete. The deadline to submit this form is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

EMINOR

You have indicated that you are an emancipated minor. We will require documentation of your emancipation. You will need to submit a copy of the court order stating you are an emancipated minor. The court order must be located in your state of legal residence. If you do not have such an order, then please contact our office in order for us to update your file.

 

 

ENTRNC

You must do an entrance interview online at www.mappingyourfuture.org. We will receive confirmation within 48 hours via the web. Completion of the interview is mandatory according to Federal law. You must complete an interview even if you have previously attended one at another school. No loans can be disbursed until you have completed the interview. If you do not have access to a computer, you may complete the entrance interview in the Financial Aid Office Resource Center.

 

EXIT

We have received information that you are either graduating or no longer attending at least part-time. Please do an on-line exit session at www.mappingyourfuture.org.

 

 

EXCESS

Based upon data provided by the National Student Loan Data System (NSLDS), the federal government has informed us that you have received a total amount in Stafford loans in excess of loan limits that have been established for the federal loan program. Your loan history will be reviewed by our office to make sure that the amounts are correct. Please note that you are not entitled to any Title IV funds until you have made satisfactory arrangements with your lender/guarantor and turn in the appropriate documentation to our office.

 

 

FEDOVR

Your financial aid is on hold due to an overpayment of a Federal Student Aid fund. You are required by law to repay any funds received from the Federal Student Aid Programs to which you are not entitled. You are ineligible to receive any financial aid until your overpayment has been repaid in full. Our office will contact the school/NSLDS to try and resolve this. However if you do owe an overpayment, we will contact you to inform you how much you must repay and to whom.

 

FERPA

We have received your request to allow a family member access to your financial aid file. You will need to fill out the FERPA form to in order for us to process your request.

 

 

HHD

Please complete the Household Size Dependent Statement form found at http://www.mscd.edu/financialaid/forms. Follow the instructions on the form carefully. You and your parents must fill it out and sign the form in ink. An incomplete form will delay the processing of your financial aid. HHI

Please complete the Household Size Independent Statement form found at http://www.mscd.edu/financialaid/forms. Follow the instructions on the form carefully. An incomplete form will delay your financial aid. The deadline to submit this form is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

 

HSTRAN

According to the Admissions Office you have not yet submitted a final High School Transcript. Federal Regulations and Metro's Admissions policy state that students must have either graduated from a high school or received a GED. Prior to our office disbursing any financial aid to your account, you MUST have your final HS transcript submitted and reviewed by the Admissions Office.

 

 

 

IVER

Your financial aid file has been selected for verification. Please complete the IVER form at http://www.mscd.edu/financialaid/forms and return it to our office. Follow the instructions on the form carefully. Many of these forms end up being returned due to no signatures, missing W2's or the packet is not fully filled out. LBYI

The results of your financial aid application indicate an exceptionally low income. Please fill out the LBYI form found at http://www.mscd.edu/financialaid/forms and return it to our office. The deadline to submit this form is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

 LGUARD

You have indicated that you are in legal guardianship. We will require documentation of your legal guardianship. You will need to submit a copy of the court order stating who your legal guardian is. The court order must be located in your state of legal residence. If you do not have such an order, then please contact our office in order for us to update your file.

 

 

 

 

LVER

Your financial aid file has been selected for verification. Please complete the LVER form at http://www.mscd.edu/financialaid/formsand return it to our office. MARGE

You must report your marital status as of the date that you signed your original FAFSA. You should not update this item if your marital status changed after you signed and submitted the FAFSA. If you made an error while filing your application, you must provide documentation of your marital status as of the date you signed and submitted your original FAFSA. Examples of this documentation include marriage certificate, divorce decree or separation documents. MCERT

Please submit a copy of your marriage certificate. If you are common-law married, you will need to provide proof - you may turn in a copy of your signed federal tax return which shows you have filed jointly. If you did not file jointly, then you must submit documentation of a joint checking account opened before the date of your FAFSA filing, a signed statement from a family member or friend that states that you represent yourselves as a married couple and you must submit a lease agreement or mortgage statement with both names. The deadline to submit this form is 2 weeks prior to the end of the term. This is due to discrepancies between what your FAFSA shows and documents you have provided our office. MVER

Your financial aid file has been selected for verification. Please complete the MVER form at http://www.mscd.edu/financialaid/forms and return it to our office. NAME

The name you have listed with our office does not match the name you have listed with the Registrar's office. Please submit the appropriate legal documentation to verify your name change and/or correction. If the difference exists because of marriage, we need a copy of your marriage certificate. If it is because of a divorce, we need a copy of the divorce decree. Should you need to correct the spelling of your name, we may only require a copy of your birth certificate or driver's license. NAMFED

Your name did not match with the Federal Processor and the Social Security Administration. Please submit a copy of your driver's license to our office. If the name is not matching due to a name change, please submit a copy of the documentation that effects the name change (i.e. marriage certificate, divorce decree, etc.). The deadline to submit this documentation is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

 

NONCIT

We are requesting documentation of your citizenship. Eligible non-citizens must submit a copy of their permanent resident alien card (front and back) or visa. U.S. citizens must submit a copy of their birth certificate or Certificate of Naturalization. You may also need to update your status with the Registrar's Office on campus. You must submit the documentation 2 weeks prior to the end of the semester you wish to receive financial aid in or risk no receiving any aid. Note: Please make sure that the photocopy shows your picture clearly. INS cannot process dark copies. The deadline to submit this documentation is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

 

 

 

ORPH

You were able to claim independency for financial aid purposes due to you being an orphan, in foster care at any time since you turned age 13 or are a ward of the court. You must submit copies of your documentation to our office. If you are an orphan, you will need to submit copies of the death certificates of your parents. If you are a ward of the court or in foster care, you will need to submit court documentation. Your court documentation must state that you were physically removed from the home through no fault of your own. Please note that if you are or were incarcerated, this does not count as documentation for independence status. If you do not qualify as either an orphan or ward of the court, you must notify our office to have your dependency status updated. The deadline to submit this documentation is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

PLDEN1

We have received the results of the credit check required for a Parent PLUS Loan. Due to regulations when your parent is not eligible for a PLUS Loan based on a Denied status you are eligible for Additional Unsubsidized loan funds with a new Stafford Loan application. Notes on your file have been created and you may call or visit our office for more information on how to apply.

 

 

PSSN

Our records indicate that there is either a discrepancy in your parent's social security number or you did not indicate their social security number on your FAFSA. This has caused your application to reject at the federal level. To correct this, we need to have the Parent Information Form completed and returned to our office. The PSSN form can be found at http://www.mscd.edu/financialaid/forms. The deadline to submit this form is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

 

RPMT

Our records indicate that you failed to successfully complete and/or withdraw from classes after financial aid paid into your student account. Due to this, you are no longer eligible for the financial aid proceeds you received. A portion of your financial aid has been canceled. You will not be eligible to receive further financial aid until your account is paid and a COPY OF THIS RECEIPT HAS BEEN SUBMITTED TO OUR OFFICE. The deadline to submit this documentation is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

 

SAPAP

You have been placed on financial aid Suspension according to our Satisfactory Academic Progress (SAP) policy. You may appeal this status. If you do not appeal your Suspension status before census date, or if your appeal is not approved, your financial aid will be cancelled. If you submit your appeal after census date and it is approved, you may only be eligible for a Federal Pell Grant and/or Federal Stafford Loans. The latest you may submit an appeal is the first day of the last month in the semester. (Example - Fall deadline is December 1st, Spring deadline is May 1st, and Summer deadline is August 1st.). SELSV

We received notification that Selective Service has not confirmed your registration or exempt status. Please submit a copy of your selective service registration acknowledgement letter, your DD214 or a copy of your registration card to our office. If you need to request a copy of your acknowledgement letter, you may call Selective Service at 1-847-688-6888 or visit their website at www.sss.gov. Males can also register over the web and print out the confirmation page for our records. Females should submit a copy of their driver's license to our office as they are exempt from this requirement. The deadline to submit this form is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

SIGNSR

When you filed your FAFSA, you and/or your parents did not submit signatures to the

Processor. You will need to sign the SAR that comes to you from CPS and return it to them. Your file has been rejected by the government and we will not be able to award you any financial aid until this has been resolved. Please submit the signatures required at www.fafsa.ed.gov.

 

 

SSNFED

Your social security number did not match with the Federal Processor and the Social Security Administration. Please turn in a copy of your social security card to our office. The deadline to submit this documentation is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid. If you have your social security card with a name different from what you are currently using, you will need to contact the Social Security Office and update your name and submit a copy of your new card with your correct name.

 

 

SSNHLD

A disbursement hold has been placed on your file due to a discrepancy between your reported Social Security Number and your Student Identification Number. You must submit a copy of your social security card for verification. The deadline to submit this documentation is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

 

SVETS

Our records indicate that you are a veteran. We have contacted Metro's Office of Veteran's Affairs to confirm your veteran's status and documentation of your veteran's benefits for the summer semester. Since we are working directly with the Veteran's office, it is not necessary for you to contact our office. Your file will be updated as soon as we receive and process the requested information.

 TAXEX

We have received your request to continue the processing of your file based on a tax extension filing.

 

TCHENT

In order to receive the Federal TEACH Grant, you must complete the Agreement to Serve at https://teach-ats.ed.gov. MSU Denver's Office of Financial Aid will be notified that you've completed your ATS.

 

 

 

TEACH

You indicated on your FAFSA that you plan to complete teacher licensure coursework. Based on this information, you may qualify for the Federal TEACH Grant program. Please note that in order to qualify for the Federal TEACH Grant program at MSU Denver; you must be pursuing licensure in one of the following areas: Secondary Education/Math, Secondary Education/Science, Special Education, K-12 Modern Languages or Elementary Education with a Linguistically Diverse Endorsement. If this is true and if you also meet the criteria listed at the bottom of this email, please stop by the Teacher Application Center at MSU Denver to complete a Pre-application form. They are located in West Classroom 136.

GPA of 3.25+ or ACT/SAT score within at least the 75th percentile in any battery At least sophomore standing (30+ credits completed/transferred) Working on your first BA/BS Citizen/eligible non-citizen Have a complete file with MSU Denver's Office of Financial Aid

Once you have completed the Pre-application form in the Teacher Application Center, you must submit the form to MSU Denver's Office of Financial Aid and attend a counseling session through Financial Aid. Upon receipt of your Pre-application form, Financial Aid will contact you directly.

 

 

TERMCO

Please log into ConnectU to read and accept the terms and conditions of your financial aid. To access the terms and conditions within ConnectU, follow these links: Financial Aid, My Award Information, Award by Aid Year (Accept Award Offers), and Terms and Conditions. Once you accept the terms and conditions of your financial aid, you will be able to accept your awards. Please contact the Office of Financial Aid if you have questions about the terms and conditions of your financial aid. The terms and conditions are a reminder of the terms you agreed to by signing the FAFSA.

 

TIVATH

Please complete the Title IV Authorization form which allows us to credit any Title IV aid to your non-institutional charges. Details of the authorization are listed on the form. The TIVATH form can be found at http://www.mscd.edu/financialaid/forms. Please complete the form and return it to our office. The deadline to submit this form is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

TIVATP

Please complete the Title IV Authorization form which allows us to credit your current Title IV aid to existing minor prior term institutional charges. Details of this requirement are included on the form. The TIVATH form can be found at http://www.mscd.edu/financialaid/forms. Please complete the form and return it to our office. The deadline to submit this form is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

 

TLIC

You have indicated on your FAFSA that you are interested in Teacher Licensure and your EFC indicates you may be eligible for a Federal Pell Grant. To be eligible for a Federal Pell Grant, you must be formally admitted to an approved Teacher Licensure Program. As of this date, we do not show you have been admitted to a Teacher Licensure Program. Please contact the Teacher Education Center in West classroom 136 or call (303) 556-3691 for information on admission.

 

 

UYOUTH

You have indicated that you received a determination any time on or after July 1, 2008 that you are an unaccompanied youth who was homeless or an unaccompanied youth providing your own living expenses. We will require documentation of your situation. You will need to fill out and submit the form UYOUTH to our office. If you are not an unaccompanied youth, please notify our office. If you feel that you should be considered to be an independent student for financial aid purposes, then please stop by our office and ask to talk to a counselor so you may explain your situation. You may also contact the National Center for Homeless Education at 1-800-308-2145 for assistance. You can find the UYOUTH form at http://www.mscd.edu/financialaid/forms.

 

 

VETERN

You have indicated that you are a veteran of active service in the US Armed Forces. Please submit a complete copy of your DD-214 (member 4) so that we may verify your veteran status. You should also contact a VA office to resolve this problem or you may be requested to turn in your DD-214 every year you apply for federal aid. The deadline to submit this documentation is 2 weeks prior to the end of the semester you wish to receive financial aid - failure to submit the documentation or submit it incomplete can result in you not being eligible for aid.

 

 

VETEST

Our office has received notification that you will be receiving veteran's benefits for the academic year. Since the Veteran's Affairs Office can not confirm the amount you will be receiving in benefits, we are estimating that you will be receiving $1531/month. Please complete the VETEST form and we will adjust your file accordingly. The VETEST form can be found at http://www.mscd.edu/financialaid/forms. The Veterans Affairs Office provides us with a copy of all certifications that occur. Please be aware that if you should certify at any point during the semester, you are obligated to notify the Office of Financial Aid.

 

 

VETS

Our records indicate that you are a veteran and you will be receiving veteran's benefits. After completing the VETS form found at http://www.mscd.edu/financialaid/forms, please bring it to the Office of Veterans Affairs located in the Central classroom building. Your file will be updated as soon as we receive and process the requested information. Keep in mind that you should always make sure that our office is aware of how much you receive in Vet's benefits. If we are not kept informed, you could possibly receive too much in aid and be billed for the excess.
