Other Insurance Participants
Patients who are not enrolled in the Metro State, UC Denver or CCD Student Health Insurance plans or Blue Cross Blue Shield plans are required to pay at the time of service. Patients who participate in the Metro State, UC Denver, CCD Student Health Insurance or Blue Cross Blue Shield Plans will have their covered charges submitted directly to their insurance companies.
We will provide clients with a HCFA form that can be filed with their insurance company. Please note that insurance companies may have guidelines and/or limitations for reimbursement.
