INSTRUCTIONS: Please complete all areas of the form below and click on "Submit" on the bottom of the page. NOTE: You'll need to bring your check, money order, or credit card (Visa, MasterCard), to the registration table in the King Center when you arrive at Achievement Day.

Each solo participant, duet team (one piano, four hands), duo team (two pianos, four hands), or chamber ensemble needs to submit a completed registration, but payments for multiple participants can be combined (note all names of registered attendees on the check).

Teachers and parents: You have no obligation to volunteer at One to One Achievement Day Saturday. If you wish to attend any of the Piano Celebration activities, please complete the separate online registration form. (Please note that the 2013 Celebration presentations are FREE of charge, but you may purchase an optional lunch.)

NOTE 1: Some Achievement Day participants will be invited to play in an early afternoon Showcase Recital at 3:00 pm! Please plan to join us!

NOTE 2: NO videotaping permitted during Celebration events; MSU Denver will be recording and retains full rights thereof.

NOTE: Free parking is available in the Seventh Street Garage (upon exit) at 7th and Lawrence Streets (855 Lawrence Way, Denver) with Piano Celebration parking voucher.
