To readers of the Change Request Chapter :

Two documents describe the questions that must be answered when submitting a change request to HLC-NCA:

• “Documentation Needed to Support a Change Request” contained in the HLC-NCA Accreditation Handbook, and
• “Special SAS Reconciliation Project.” Questions created by HLC for institutions that are already able to offer programs online.

In the Change Request Chapter, the headings in the shaded areas and questions in boxes indicate topics and questions that are part of the HLC-NCA Accreditation Handbook. Items in bold refer to questions asked in the second document – the Reconciliation Project. The responses are combined.
