Loan Discharge Application Due to Disability

If you have been deemed totally and permanently disabled by a LICENSED PHYSICIAN, you may qualify to have your loan discharged.

Request for Forbearance

If you are unable to make payments, but do not qualify for deferment, you may place your account into a forbearance. You are eligible to use up to 36 months of forbearance (awarded only in 12 month increments). Please be aware that your account accrues interest while in a forbearance status.

Request for Economic Hardship

If you are unable to make payments and you feel that you qualify for economic hardship deferment, please use this form. Supporting documentation MUST be submitted with your claim. Please read the qualifications list on the form or your promissory note to see if you qualify. Borrowers are only eligible for 36 months of economic hardship over the life of their loan (awarded only in 12 month increments).

Request for Cancellation

One of the main benefits of the Perkins loan is that it may be cancelled. If you are employed in certain fields such as; Health Care, Teaching, Family Services, Law Enforcement… you may be eligible to cancel your loan in its’ entirety. Please see your promissory note for specific qualifications. In all cases supporting documentation MUST be submitted to qualify for cancellation.

Unemployment Deferment Form

If you are unable to make payments and you feel that you qualify for unemployment deferment please use this form. Supporting documentation MUST be submitted with your claim. Please read this form or your promissory note to see if you qualify.
