The Measles, Mumps and Rubella
(MMR), Meningitis and TB Risk Assessment

Measles Mumps and Rubella Requirement
Students born after January 1, 1957 must provide documented proof that they have received two rubeola (measles), two rubella (German measles) and two mumps vaccinations OR two combination MMR (measles, mumps and rubella.)*
Students born before January 1, 1957 must provide documented proof that they have received one rubeola (measles), one rubella (German measles) and one mumps vaccination OR one combination MMR (measles, mumps and rubella.)*
*An alternative option is to provide written evidence of laboratory tests (known as a blood titer tests for immunity) showing immunity to measles, mumps and rubella for those who have previously had each of these diseases. If you cannot provide proof of immunization or positive blood titer tests, you will have to be re-immunized.
The Meningococcal Disease Information Requirement
State Law requires all post secondary education institutions to provide information regarding Meningococcal Disease to students. There is scientific evidence that college students living in dormitory facilities (communal environments) or congregate settings are at a higher risk of contracting this serious disease. There is an immunization available that can prevent against four types of Meningococcal disease. Although the State requirement is only to inform students about Meningococcal disease and the associated vaccine, the Health Center at Auraria highly recommends students at high risk consider being immunized against this disease. This immunization is available for $115.00 at the Auraria Immunization office or you may contact your private physician for more information. All students must sign and submit the meningococcal disease information sheet acknowledging that they have received this critical information.
Tuberculosis (TB) Risk Assessment Requirement
All new students are required to complete a TB risk assessment questionnaire. These will be reviewed by the medical staff at the Health Center at Auraria. The cost for this test is $10.00. For more information on Tuberculosis click HERE
PPD (Tuberculosis) Testing is available at the Immunization office for $10.00. PPD testing is done on Monday, Tuesday and Wednesday 8:00 a.m. - 12 p.m.1-4:30 p.m. and on Friday 8:00 a.m. - 12:00 p.m. and 1:00 - 2:30.
Participation in academic programs such as Teacher Education or Nursing may require students to be tested for Latent Tuberculosis. This test (PPD) requires students to report for test placement on their forearm and to return in 48 hours to have a medical professional read and record the result of the test. Once this is completed the student will receive a result document which can be submitted to their program.
Compliance Deadline for First time students registering for either the Summer or Fall 2009 semesters
Proof of immunization must be on file by no later than August 1, 2009. Administrative holds will be placed on all student accounts that are not in compliance by the August 1, 2009 deadline. These holds will prevent Summer 2009 registration and future registrations until immunization compliance is achieved.
Students are encouraged to submit their immunization documentation immediately to ensure adequate processing time. Documents must be turned in to the Immunization Office and not through Admissions. |
Required Documents
You must download, print and complete forms one (1), two (2), and three (3) below and submit them along with your proof of immunization. All four (4) documents (forms one, two and three and your proof of immunization) must be submitted at the same time in one sealed envelope. Once your documentation is received and the Immunization Office determines you have met all immunization requirements, you will not need to provide this information in future semesters.

OR hand deliver records during normal business hours to the Immunization Office, Plaza 149. After hours you may drop documents that are in a sealed envelope in the blue immunization drop box outside of the Health Center at Auraria’s front door located in the Plaza Building, room 150.
DO NOT deliver records under either of the locations door.
*Please make a copy of all submitted documents for your personal records so that you have a copy in the event that your paperwork does not reach the Immunization Office. |

Select only one of the options below for submitting the required Proof of Immunization:
1. Certificate of Immunization (PDF) signed by a physician, nurse or school authority. This form can be taken to a medical provider or agency that has documented records of your immunization history. They will transfer your immunization records to this form. There is no need to print this PDF form if you select to submit your proof of immunization using one of the other options listed below. This form is also used for exemption purposes."
*Please note that in the case of a campus closure ordered by the State Health Department (due to an outbreak of disease) students filing exemptions will not be permitted to attend classes or services on campus and my not be reimbursed tuition and fees.
2. Copy of Medical records from a physician which outlines your immunization history. This information is kept by a family physician, pediatrician or State/County immunization clinics where you received health care.
3. Proof of Personal History of measles, mumps and rubella which can clinically be determined through a blood titer test which assesses immunity. If you believe you had each of these diseases, your physician can order blood tests known as blood titers. Only a positive result for all three (3)diseases (measles, mumps and rubella) will be accepted as proof of immunity. Consult your physician or a medical representative from the Auraria Immunization clinic for further guidance.
4. Official International "Yellow" Certificate of Immunization: This document is provided by travel clinics, State and County immunization clinics and often carried by Military members from the Department of Defense. This certificate is yellow and may include information such as blood type and allergies. This is often required to enter foreign countries.
5. Official signed copy of High School Immunization Record: Any student who has attended elementary or secondary education in the state of Colorado is required to provide the public school system with evidence of childhood immunizations. Historically, vaccine records were kept on cards and maintained by a school medical official. More recently, some schools have transcribed immunization records into a school data base and are reported with a student's academic records. An individual may request a copy of their official transcript and immunization record from the high school they attended. Students who attended public school systems in other states are encouraged to inquire about personal records from the school they attended.
Adult Shot Record and Immunization Recommendations
Although documentation of MMR is the only immunization requirement to attend college, it is requested that you provide dates of additional immunizations with the proof of immunization that you file. The Centers for Disease Control and Prevention (CDC) reviews and recommends adult immunizations annually and you may not have protection against diseases that are now preventable through Tetanus Diphtheria and Pertussis (Tdap), Polio (IPV), Meningitis, Varicella (Chickenpox) and Human Papilloma Virus (HPV)
(Gardasil vaccine against cervical cancer). You may also go to the CDC website for more information at http://www.cdc.gov .
Cost of Immunizations and Where to obtain Immunizations
Students can obtain immunizations from their community medical provider or from the Health Center at Auraria's Immunization Office. Community prices for MMR vaccine and blood titer tests for immunity may vary widely from one office to the next.
The Health Center at Auraria has established special immunization pricing below community standards in an effort to control costs to students needing immunization services. The cost of one MMR shot is $14.00 ($28.00 for those needing two shots). The cost of a blood titer test for immunity is $25.00 per disease or $75.00 for measles, mumps and rubella titers. There are no additional charges for administering shots or drawing blood and no office visit charges will be assessed for routine immunization services. Please be aware that when utilizing community medical providers these additional charges are often assessed, in addition to the cost of the vaccine or blood draw.
Optional Free HIV Testing is available
Optional confidential HIV testing is free of charge at tech Health Center at Auraria. HIV testing is done Monday - Friday upon request during normal business hours.
Auraria Immunization Office Location and Hours of Operation
The Immunization Office is located at the Health Center at Auraria on the Auraria Campus, Plaza Building, Suite 149. Immunizations are offered Monday, Tuesday, Wednesday and Thursday 8:00 a.m. until 12:00 p.m. and 1:00 - 4:30 p.m. and Friday 8:00 a.m. to 12:00 p.m. and 1:00 p.m. - 2:30 p.m.
The
