This form is intended only for use by students already registered with the Access Center. This form needs to be submited each semester you are requesting accommodations.
Our office can only provide accommodations for Metro courses. If you are taking a UCD or CCD course, you will need to request accommodations through each institutions respective office. If you are requesting Alternative Text for this semester, please fill out and submit the respective request form. If you have any questions or concerns regarding your accommodations please contact our office to set up a meeting with your coordinator.
